The Director of the
Administrative Office has revised Director's Procedural Forms 254,
255, and 256, the bankruptcy subpoenas, to incorporate the changes
in Civil Rule 45 which were effective on December 1, 2006. The
changes in the three forms were effective on December 1, 2006.
The Director has
revised and reformatted the Reaffirmation Agreement and Order as two
forms. The new Director's Procedural Forms 240A, Reaffirmation
Agreement, and 240B, Order on Reaffirmation Agreement, will be
effective on January 1, 2007. Page 1 and Part D of Form 240A were
reformatted and Form 240B, the proposed order, was revised to
include several alternative provisions. Amended Director's
Procedural Form 104, Adversary Proceeding Cover Sheet, and Form 210,
Transfer of Claim Other Than for Security, will be effective as the
bankruptcy courts implement CM/ECF Release 3.1. Most bankruptcy
courts are expected to have implemented Release 3.1 by October 17,
2006. In addition, Form 200 has been amended to incorporate the
October 1, 2006, changes in Official Form 23 and Interim Bankruptcy
Rule 1007. New Director's Procedural Form 202, Statement of Military
Service, and amendments to Director's Procedural Forms 240,
Reaffirmation Agreement; 271, Final Decree; and 281, Appearance of
Child Support Creditor or Representative, were effective on August
1, 2006.
